Medication Management: Dispense drugs prescribed by providers and counsel patients on their proper use
Clinical Consultation: Advise physicians and healthcare teams on medication selection, dosage, interactions, and side effects
Order Review: Evaluate provider orders for duplications, incompatibilities, allergies, and interactions; recommend safe alternatives
Patient Counseling: Provide information on prescription medications, adverse reactions, and use of home healthcare supplies
Recordkeeping: Enter and verify all medication orders in the VA’s VistA electronic patient record system
Controlled Substances Oversight: Maintain strict compliance with federal and state regulations, safeguarding and documenting all controlled substances

Verification of pharmacist licensure (full, current, unrestricted)
Education: Graduate of an ACPE-accredited College or School of Pharmacy with a B.S. Pharmacy or Pharm.D. degree
Licensure: Full, current, and unrestricted license to practice pharmacy in any U.S. state, territory, or DC, including controlled substance authorization
Experience: Demonstrated proficiency in clinical decision-making, prescription review, and patient counseling
Skills: Fluent in English; ability to document, communicate, and collaborate effectively
Compliance: Must adhere to facility by-laws, medical staff rules, and VA pharmacy standards
