Perform Biopsychosocial Assessments and plan interventions to address social determinants of health.
Conduct Suicide Risk, Mental Health, and Crisis Assessments and interventions.
Document safety plans, plans of care, and all patient interactions in the medical record.
Assist with discharge planning, including referrals to rehabilitation, long-term care, group homes, or home health services.
Provide resources for veterans experiencing homelessness or abuse/neglect.
Collaborate with interdisciplinary teams and schedule family or goal-of-care meetings.
Offer guidance and completion of advanced directives.
Address patient transportation needs and assess post-discharge case management services.
License: Active, full, and unrestricted LCSW license in any U.S. state, territory, or commonwealth.
Technical Proficiency: Must be fluent in English (spoken, read, and written).
Physical Ability: Ability to lift up to 50-100 lbs., assist in moving patients, and handle extensive walking/standing.
Minimum of 2 years of recent experience in social work; VA experience is required.
